Hint: The easiest way to use this form is to select the items you want, add them to your shopping cart, view the cart, and then print it out. You can then attach the printout to the paper order form. That way, all you need to fill in on the order form is your Billing and Shipping information. Once you have printed out the cart, you can empty it.
